This lecture by Dr. William Tamborlane, MD describes the potential benefits of real-time continuous glucose monitoring (RT-CGM) in the management of patients with type 1 diabetes (T1D). It reviews the evidence from randomized clinical trials that has been incorporated into the American Diabetes Association guidelines for use of these devices in clinical practice. Recent studies have also demonstrated improved outcomes when CGM is combined with insulin pump therapy in patients who are unable to achieve target A1c levels with glargine-based multiple daily injection regimens.

Goal and Objective
After participating in this activity, the viewer should be better able to:
1. To know how real time continuous glucose monitoring (RT-CGM) can be utilized to improve basal/bolus insulin therapy in type 1 diabetes (T1D).
2. To recognize that A1c levels can be reduced with RT-CGM in adults and in selected children with T1D in whom A1c levels are elevated, if they utilize the devices on a nearly daily basis.
3. To know that RT-CGM can also benefit patients with T1D who have A1c levels < 7.0% by helping them to maintain target A1c levels with reduced biochemical hypoglycemia.
4. To know how patients perceive the benefits and hassles of using current CGM devices.
5. To be aware that improved control of diabetes can be achieved by the nearly simultaneous introduction of CGM combined with insulin pump therapy in patients who are unable to achieve target A1c levels with glargine-based multiple daily injection regimen
