This lecture by Ronald A. Sage, presents the indications, technique and complications associated with the Syme's Transmalleolar Amputation. The procedure is compared to other partial foot amputation techniques, and the possible complications of these various techniques are reviewed. Emphasis is placed on pre-surgical criteria for distal wound healing that should be used in making the decision to proceed with a Syme procedure. Possible causes of limb salvage failure besides breeching traditional criteria are reviewed. An effort is made to assist the listener in selecting patients with seriously infected or necrotic diabetic feet who would likely benefit from a distal amputation attempt versus a higher transtibial or transfemoral amputation. Outcome studies citing the speaker's experience with this procedure are presented.
